The goal is to reduce the total cost for all parties through a wellness and disease management approach.
Some of the diseases well suited to proactive management are Diabetes, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and Coronary Artery Disease (CAD). It has been demonstrated that by routine monitoring and early intervention when so indicated, Emergency Room visits and Hospitalizations can be reduced by 50% or more. For CHF alone, reducing hospital and ER events is estimated to generate a healthcare costs savings of $4 billion per year in the United States.

Our approach We offer a unique approach by working with the healthcare provider organizations to acheive optimal home health. We idenify open standards based technologies to keep costs low and adopt new emerging technologies. For the provider organization we can: - Help source the technology for the patients home including the appropriate healthcare appliances. - Identify a solution that can be tailored to patient and HCP needs - Deliver training for the home health provider organization - Support the opportunity to integrate with healthcare providers Electronic Medical Records (EMR/EHR) -Encourage patient reminders for medications and treatments |
What are the compelling reasons that this is the time to commit?
1. New technologies for the home are evolving based on an open standard hubs and compatible wireless devices. Within the next year, technology will grow significantly in terms of the devices that will connect the healthcare providers. Smart phones and tablet devices are now making ease of use for patients more apparent. This will give us the ability to apply these technologies to reduce the total cost of healthcare.
2. CMS has stated that over $17 billion is spent on excessive avoidable readmissions. In 2012 CMS institued a program to penailize organizations for excessive readmission rates, which could cost up to $125,00 per organization in reduced reimbursements.
3. Published reports from organizations such as Geisinger, Partners and Spectrum Health are documenting the impact mobile technology is having in reducing readmissions by 50% or more. The VA has stated telehealth is reducing hopital admissions by 30% and saves the VA $1999 per year per patient. Utilization of data collected from these programs provides opportunities for better understanding diseases and the appropriate treatment regimens.
4. Healthcare is clearly becoming more digital as Hospitals and Physician Practices are automating the flow of patient information through the adoption of EMR system based on incentives under the Affordable Care Act. A report from HIMSS stated 89% of doctors use laptops for daily clinical activity.
